Conflict of Interest Disclosure Form
In accordance with WJCS’s Conflict of Interest Policy, a conflict of interest is defined as: a circumstance, condition or situation in which your personal interest interferes with the performance of your responsibilities in ways which may result in personal, financial, professional and/or political gain at the expense of or to the detriment of WJCS and/or the individuals served.
A conflict of interest may occur if your outside activities or personal interests influence or give the appearance of having influence over your ability to make objective decisions in the course of performing your job responsibilities.
Please complete and return the conflict of interest disclosure statement. Please be assured that the disclosure requirements are intended to provide the Board and Management with a systematic and ongoing method of disclosing and ethically resolving potential conflicts of interest. Although it is impossible to list every circumstance giving rise to a possible conflict of interest, the following will serve as a guide to the types of activities that might cause conflicts and that should be fully reported:
For purposes of this policy, “family members” include one’s parents, spouse, domestic partner, brothers, sisters, children, grandchildren, great-grandchildren, spouses of children, grandchildren or great-grandchildren, relatives supported financially, father-in-law, mother-in-law, sister-in-law, and brother-in-law.
1. Outside Interests
a) To hold a direct or indirect material financial interest in any outside concern from which WJCS secures goods or services.
b) To compete, directly or indirectly, with WJCS in the purchase or sale of property or property rights, interests, or services.
2. Outside Activities
To render directive, managerial, or consultative services to any outside concern that does business with, or competes with the services of WJCS or to render other services in competition with WJCS.
3. Inside Information
To disclose or use information relating to WJCS’s business for the personal profit or advantage of the individual or his/her immediate family.
4. Gifts, Gratuities, and Entertainment
To accept gifts, excessive entertainment, or other favors from any outside concern that does, or is seeking to do, business with, or is a competitor of, WJCS – under circumstances from which it might be inferred that such action was intended to influence or possibly would influence the individual in the performance of his/her duties.
Each staff member, board member, contractor, consultant, and volunteer has a responsibility to disclose any actual or suspected conflict of interest to the CEO or Board President and the Compliance Officer. Following a review of the facts a determination will be made as to whether a conflict of interest exists and the situation will be addressed in accordance with agency policy.
