WJCS is currently searching for a temporary, 21 hours per week, Waiver Service Coordinator to work throughout the Westchester area.  The Waiver Service Coordinator, under the NHTD/TBI Waiver, uses a person-centered planning process to develop, implement, and maintain the Service Plans, makes referrals, conducts meetings, prepares and distributes reports as required, and ensures that Medicaid and other government benefits are maintained for assigned individuals. Candidate must have good oral and written communication skills, ability to develop rapport with individuals served and their families, strong attention to detail and is able to multitask efficiently and effectively.  Knowledge of NHTD Waiver/TBI Waiver services, DOH regulations and government benefits, such as Medicaid and SNAP a plus.

The ideal candidate must possess a minimum of a Bachelor’s Degree and 3 years case management/service coordination experience along with linking and referring to community based services for individuals with disabilities and/or seniors. Candidate must have a Valid New York State driver’s license and a vehicle to travel from one site to another.
