Pathways to Care (End of Life Care)
Helping individuals and families address the challenges of serious illness
Life threatening illness takes its toll not only on a patient but also on the patient's family. With funding from UJA-Federation of New York, PATHWAYS TO CARE provides compassionate individual and family counseling, homecare assistance and supportive connections within a Jewish context when people and their loved ones face terminal illness.
PATHWAYS TO CARE offers an array of services including:
• Support and Care Management
• End of Life Care
• Spiritual Counseling
• Bereavement Support Groups for individuals, families and communities
• Individual Grief Counseling and Psychotherapy
• On-Site Grief Counseling
PATHWAYS TO CARE also connects families to professionals who can give advice about critical decisions in a variety of areas related to personal, legal, health, insurance and entitlement issues.
