Coordinated Special Care Treatment for Young Adults

If you’re an adolescent or young adult who has recently begun withdrawing from friends and family, had unusual thoughts or beliefs, acted strangely, or become fearful or suspicious of others, OnTrack NY is an innovative treatment program that can provide support and help.

Tailored for individuals age 16-30, the OnTrackNY team at WJCS serves individuals and their families in Westchester, Putnam, and Rockland Counties. Our Coordinated Specialty Care Team includes a Peer Specialist, Supported Education and Employment Specialist, Primary Clinician, Nurse, Prescriber and Team Leader. We provide counseling, social skill-building, vocational support, health and wellness coaching, and resources to help recovery. In addition to providing services in the WJCS Peekskill and Hartsdale Clinics, we can also meet participants in the community and via Telehealth, including video chat and phone appointments.

The WJCS OnTrackNY team provide services to all referred individuals meeting clinical admission criteria, which is determined after meeting with the individual and completing an assessment, without wait lists and regardless of their insurance or ability to pay.

Disturbing thoughts, feelings and behaviors, if left untreated, can become worse over time. Let’s work together on reducing your symptoms so you can achieve your interpersonal, academic, and vocational goals.
