WJCS Certified Community Behavioral Health Clinics

WJCS Certified Community Behavioral Health Clinics (CCBHC) in Hartsdale, Peekskill, Mount Vernon, and Yonkers provide integrative services to clients who are living with a variety of behavioral health conditions, including but not limited to serious mental illness, substance use disorders, and serious emotional disturbance.

The CCBHC Services include the following:

• Screening, assessment, and diagnosis, including risk assessment

• Patient-centered treatment planning, including crisis planning

• Comprehensive outpatient mental health and substance use services

• Case management

• Community-based recovery supports, including peer support, counselor services, and family supports

• Crisis mental health services, including 24-hour mobile crisis team emergency crisis intervention services, and crisis stabilization

• Screening for HIV and viral hepatitis (A, B, and C) risk, as well as referrals for Hepatitis vaccinations and further assessment where indicated

• Outpatient primary care screening and monitoring of key health indicators and health risk

• Clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome consistent with published guidelines

• Psychiatric rehabilitation services

• Intensive community-based mental health care for veterans and their families

• Employment assistance and coaching

All CCBHC clients will have access to these services, regardless of their ability to pay or insurance status.

Q: What is a CCBHC?

A: CCBHC stands for Certified Community Behavioral Health Clinic, and it is a federal designation for behavioral health providers who offer a comprehensive set of services in alignment with federal standards.

Q: Who is most appropriate for CCBHC services at WJCS?

A: The CCBHC program offers a comprehensive range of services. It is very helpful for individuals who have mental health and/or substance use disorders, along with other co-occurring issues, including chronic health conditions and social determinants of health (i.e., housing instability, unemployment, lack of social connectivity). However, anyone living with a behavioral health condition is welcome to enroll in the program if they can benefit from the services offered by the CCBHC.

Q: How is this different than the services WJCS has previously provided?

A: The CCBHC model allows WJCS to provide more holistic services and supports. It integrates behavioral health care with support for physical health and social determinants of health-related issues, and care coordination for all. As a CCBHC provider, we are funded to provide the services we know our clients need, but that have been traditionally underfunded within behavioral health clinics to date.
