WJCS Certified Community Behavioral Health Clinics (CCBHC) in Hartsdale, Peekskill, Mount Vernon, and Yonkers provide integrative services to clients who are living with a variety of behavioral health conditions, including but not limited to serious mental illness, substance use disorders, and serious emotional disturbance.
All CCBHC clients will have access to these services, regardless of their ability to pay or insurance status:
- Screening, assessment, and diagnosis, including risk assessment
- Patient-centered treatment planning, including crisis planning
- Comprehensive outpatient mental health and substance use services
- Case management
- Community-based recovery supports, including peer support, counselor services, and family supports
- Crisis mental health services, including 24-hour mobile crisis team emergency crisis intervention services, and crisis stabilization
- Screening for HIV and viral hepatitis (A, B, and C) risk, as well as referrals for Hepatitis vaccinations and further assessment where indicated
- Outpatient primary care screening and monitoring of key health indicators and health risk
- Clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome consistent with published guidelines
- Psychiatric rehabilitation services
- Intensive community-based mental health care for veterans and their families
- Employment assistance and coaching
