Sometimes there are things that affect our health that can’t be fixed by doctors or medical treatment. UJA SocialRx connects isolated older adults to valuable community resources that improve overall health and well-being.

In partnership with the JCC Mid-Westchester, a friendly connection specialist helps identify resources to deal with tough challenges like loss, life transitions, chronic health issues, financial stress, or loneliness.

By focusing on social health, connection specialists foster meaningful relationships that can improve overall well-being.

The connection specialist listens to what is important and makes referrals to community programs and services Including exercise, art therapy, creative workshops, volunteering opportunities, support groups, and others.

Accessing services is less overwhelming when there is a caring professional providing support at each step of the way.

Through this free program offered in partnership with the JCC Mid-Westchester, our warm and friendly connection specialist will work with isolated older adults to assess their needs and connect them to an array of community-based services, including:

  • Home-delivered meals
  • Transportation
  • Vaccine appointments
  • Ongoing case-management programs
  • Home-based medical services
  • Telephone or in-person counseling
  • Home care
  • Social programming
