Home Health Services of WJCS offers a broad range of home care services including:
- Medical & Social Appointments
- Assisting with Ambulation Transferring
- Meal Planning
- Medication Reminders
- Range of Motion Exercises
- Light Housekeeping
Home health aides with specialty training in the areas of cancer and depression & mental health, dementia/Alzheimer’s and end of life care are available upon request.
Additional Home Health Services
Additional services include: nursing assessment, referrals to occupational, physical and respiratory therapy, case management, and companion and socialization services for people with special needs who reside in nursing homes, hospitals and senior citizen homes. Referrals for services other than those provided by Home Health Services of WJCS can be arranged and facilitated by nursing and/or case manager staff.
Through the Department of Health's Traumatic Brain Injury (TBI) and Nursing Home Transition and Diversion (NHTD) Waiver Program, WJCS offers home care support to adults whose TBI occurred after the age of 21 or individuals who meet nursing home criteria but prefer to live in the comfort and security of their own home.
