Care Coordinator

WJCS is currently searching for a full-time, 35 hours per week, Care Coordinator to work in the Health Home Program throughout Westchester County with our office based in Yonkers, NY. Under supervision of the Care Manager, the Care Coordinator will work with individuals in the community who have two chronic medical conditions or a mental health issue.  The goal of the program is to assist these individuals in managing all of their health needs and in living a well-balanced, health life.

Responsibilities include providing care management services and health coordination to health home members; providing timely outreach and engagement to potential health home enrollees as identified by the Health Home; overseeing and providing access to the services an individual needs with the goal of maintaining health and avoiding unnecessary use of hospitals and emergency rooms.

The ideal candidate must have knowledge of serious health and mental health conditions and management of same; knowledge of community resources and advocacy; and must be able to communicate effectively in English both written and orally. Bi-lingual, English/Spanish is required. Must possess a bachelor’s degree with two (2) years of health care experience or experience working with medically underserved population. Must possess a valid NYS driver’s license and have the ability to travel to various communities and meeting sites.

Community Direct Support Professional

Community Habilitation Worker

The ideal candidate must possess a High School Diploma or equivalent. Must have knowledge of basic residential services; able to handle sensitive issues while protecting others’ welfare; and able to participate in, pass and maintain certification of WJCS required training that includes but may not be limited to Medication Administration, SCIP, CPR/First Aid, Driving the Van and Mandatory Training as identified by OPWDD. The ideal candidate must possess a Master’s degree in Social Work from an accredited school of social work and have a license (LMSW or LCSW) to practice social work in New York State issued by the State Education Department. Registered Nurse

We are searching for full-time and part-time Registered Nurses, to work in our groups homes located in Westchester County. Under general supervision from the Director of Nursing Services, the Registered Nurse will provide nursing services for individuals residing in the homes operated by Community Programs of WJCS.

The Registered Nurse will promote optimal health and safety through assessment, planning and intervention as well as collaboration with an interdisciplinary team and other health professionals. Other responsibilities include coordinating mandated evaluations, such as physicals, dental, eye/vision, audio evaluations, and provide follow up for all medical recommendations; collaborating with the IDT to promote the strengths of each individual, as well as reports on current health status at quarterly, semi- annual and annual reviews; and provides training to staff on all medical protocols and ensures that all staff have a good working knowledge of the all protocol and ensures documentation is present for all trainings.

The ideal candidate must possess a valid New York State license to practice as a Registered Nurse; valid driver’s license and vehicle required; have a CPR Certification; and have experience working with the ID/DD population. Must possess good interpersonal and organizational skills and be able to communicate effectively in English, both orally and in writing. Working knowledge of OPWDD regulations as it relates to nursing practices and procedures helpful.

BA degree preferable with LPN or RN License, two years supervisory experience and three years’ experience working with individuals with intellectual/developmental disabilities. Social Club Assistant

WJCS is currently seeking both permanent part-time and per diem staff to work in our Social Clubs located in Peekskill and Shrub Oak, NY. Positions are open for Tuesday/Thursday from 5pm-9pm and daytime openings for Monday, Wednesday, and Thursday from 8-2pm.

WJCS is currently seeking a part time, 18-24 hours per week, Coordinator to work in three of our Social Clubs in Mohegan Lake, NY. Under the general supervision of the Director of Mental Health Recovery Services, the Coordinator is responsible for providing administrative, fiscal and programmatic supervision for three social clubs serving individuals with mental illness and programmatic responsibility for two social clubs serving individuals with mental illness. At each of the three clubs, the Coordinator is responsible for developing a program structure and environment that encourages and assists persons to further their recovery in areas of their own choosing including, community integration, education, housing, spiritualism, personal growth, volunteerism, work, and relationships. Other responsibilities include leading wellness groups; fostering and assisting club members to access information, resources, and engage in behavior that contribute to achieving their recovery goals; and preparing monthly management and financial information reports.    

The ideal candidate possess a bachelor’s degree in human services; experience working with individuals with SPMI; and experience with budget preparation. Must be able to model recovery behaviors and have a valid driver's license and a safe driving record and ability to safely operate agency vehicles, including 12 passenger vans. Experience in person-centered recovery a plus.

WJCS is searching for a full-time, 35 hours per week, Social Worker to work in the Peekskill general mental health clinic (21 hours) and in Peekskill’s Trager-Lemp Center Treating Trauma Promoting Resilience (14 hours). Under the general supervision of the Clinic Director/Assistant Clinic Director, the Social Worker will provide mental health services to persons in the community having psychiatric diagnoses and requiring services offered by a licensed mental health facility. Under the general supervision of the Trager-Lemp Center Team Supervisor, the Social Worker will provide services to persons in the community who have been affected by sexual abuse or family violence as a victim, or family member of the same. SPANISH SPEAKING STRONGLY PREFERRED.

Responsibilities include providing direct service in various therapeutic modalities, including assessment, crisis intervention, and ongoing treatment; responsible for all recording and statistical procedures required by the agency including case records, insurance forms and related data, monthly statistical sheets schedules, surveys, etc.; and maintaining necessary patient related contacts with other agencies and resources (courts, schools, etc.) including written material, telephone contact and personal visits as necessary. Other responsibilities may include supervising other staff or students, when applicable; filing insurance claim forms and maintaining other necessary contacts with insurance companies; and providing emergency and general clinical coverage for the Peekskill Clinic. The Social Worker will receive on-going clinical supervision and training in the WJCS Educational Institute. LMSW required and two evenings (12-8) required. Knowledge of evidence-based principles and techniques is a plus as well as comfort with computer applications and good knowledge of community resources.

The ideal candidate must possess a Master’s degree in Social Work from an accredited school of social work. Must have thorough knowledge of modern principles and techniques of social work and therapeutic interventions with individuals and families.

WJCS is currently searching for a Social Worker for the Linkages Program serving Westchester County. The Social Worker will work directly with youth involved in the juvenile justice system, specifically, the Department of Probation, throughout Westchester County. The selected individual will primarily complete comprehensive, forensic, mental health assessments and referral for mental health/substance abuse treatment and other community based services, for clients assigned to WJCS' Linkages Program.

This is a full-time, 35 hours per week, position. The selected candidate must be a NY Licensed Masters Level Social Worker or NY Licensed in a related field from an accredited school, plus two years experience working with adolescents.  The selected individual must also have a strong understanding of adolescent development, be able to work well independently, have strong written and oral communication skills and be able to work in various host locations.  Licensure will be required within three months of hire.  Bi-Lingual English/Spanish a plus.  The selected person must have their own motor vehicle in order to travel throughout Westchester County.
