Service Coordination
SERVICE COORDINATION is provided to children and adults with mental retardation, a developmental disability, or who have a special need and who live in a certified residence, with family, or independently.
The Service Coordinator is responsible for assessing the individual's needs, developing a service plan, linking the person to appropriate services and advocating for services and benefits. In addition, the service coordinator develops and maintains a close relationship with the individual and their family as well as their school or employer and other service providers. This service may be funded through Medicaid or a private pay arrangement.
