Pathways to Care (End of Life Care)
Helping individuals and families address the challenges of serious illness
Life threatening illness takes its toll not only on a patient but also on the patient's family. With funding from UJA-Federation of New York, PATHWAYS TO CARE provides compassionate individual and family counseling, homecare assistance and supportive connections within a Jewish context when people and their loved ones face terminal illness.
PATHWAYS TO CARE offers an array of services including:
- Support and Care Management
- End of Life Care
- Spiritual Counseling
- Bereavement Support Groups for individuals, families and communities
- Support Group for Chronic Illness
- Individual Grief Counseling and Psychotherapy
- On-Site Grief Counseling
UJA-Federation funding also supports a Pathways to Care/Beth Israel Medical Center collaboration to provide Westchester physicians with pain management and palliative care consultative services. As part of the collaboration, Dr. Russell Portenoy, chairman of the Department of Pain Medicine and Palliative Care at Beth Israel, is available for physician-to-physicians consultations. Additionally, various seminars for local physicians will be sponsored and information disseminated about the Palliative Care Information Act, that went into effect earlier this year. PATHWAYS TO CARE also connects families to professionals who can give advice about critical decisions in a variety of areas related to personal, legal, health, insurance and entitlement issues.
