Care Coordinator

WJCS is currently searching for a full-time, 35 hours per week, Care Coordinator to work in the Health Home Program throughout Westchester County with our office based in Yonkers, NY. Under supervision of the Care Manager, the Care Coordinator will work with individuals in the community who have two chronic medical conditions or a mental health issue. The goal of the program is to assist these individuals in managing all of their health needs and in living a well-balanced, health life.

Responsibilities include providing care management services and health coordination to health home members; providing timely outreach and engagement to potential health home enrollees as identified by the Health Home; overseeing and providing access to the services an individual needs with the goal of maintaining health and avoiding unnecessary use of hospitals and emergency rooms.

The ideal candidate must have knowledge of serious health and mental health conditions and management of same; knowledge of community resources and advocacy; and must be able to communicate effectively in English both written and orally. Must possess a bachelor’s degree with two (2) years of health care experience or experience working with medically underserved population. Must possess a valid NYS driver’s license and have the ability to travel to various communities and meeting sites.

Community Direct Support Professionals

Community Programs of WJCS is currently searching for two full-time, 40 hours per week, Community Direct Support Professionals (DSP) to work in our Community Engagement Program. Under the general supervision of the Employment Coordinator, the Community DSP assists in supporting individuals with disabilities in the community in leisure activities, internships and job placements. Community Habilitation Worker

WJCS is currently searching for several Community Habilitation Trainer to work up to 20 hours per week at $14 per hour. The Community Habilitation Trainer will work with individuals in their apartments in White Plains. Under the general supervision of the POINT Director, the Community Habilitation Trainer will be responsible to provide in home and community based habilitation services such as education, instruction, and direct services to individuals with MR/DD/LD in supported independent living settings. Other responsibilities include assisting with the development of the individuals’ Community Habilitation Plans; promoting individual’s chosen individualized outcomes that empower and enable individuals to achieve their stated goals and to live as optimally as possible; and assisting in the development of social skills by providing education, training, and role modeling.

The ideal candidate must possess a High School Diploma or equivalent with one year experience working in a related field. Experience in working with the DD population; good knowledge of nutrition and good financial practices; and able to communicate effectively in English, both orally and in writing. Candidates must maintain reliable transportation and possess a valid NYS driver’s license and a driver’s record considered acceptable to agency insurance carrier.

Direct Support Professional(s)

Community Programs of WJCS is currently searching for both full-time (40 hours/week) and part-time (12-35 hours/week) Direct Support Professionals (DSP) to work in one of the 13 group homes located in Westchester County. Under the general supervision of the Residence Manager, the DSP will support intellectually/developmentally disabled individuals in their development of basic living and social skills with the primary goal of integrating them into the community to the best of their ability. Other responsibilities include providing assistance, services and supports that promote independence and active participation in daily living within the residence and community; administering medications as assigned; and observing and reporting individuals’ illness, health care concerns, injuries, incidents and possible individual abuse.

The ideal candidate must possess a High School Diploma or equivalent. Must have knowledge of basic residential services; able to handle sensitive issues while protecting others’ welfare; and able to participate in, pass and maintain certification of WJCS required training that includes but may not be limited to Medication Administration, SCIP, CPR/First Aid, Driving the Van and Mandatory Training as identified by OPWDD. Candidates must have a valid NYS driver’s license for at least two years. A minimum 6-12 months working with people with developmental disabilities, preferred. This position requires physical activity including but limited to lifting, moving, pulling, pushing, flexing, and bending.

Director of Nursing Services

Westchester Jewish Community Services (WJCS) has an opening for a full-time, 35 hours per week, Director of Nursing Services to work in our Community Residential Care Division in White Plains, NY. Under the general supervision of the Assistant Executive Director, the Director of Nursing Services is responsible for supervising the nursing service in the residences. Other responsibilities include coordinating the development and implementation of nursing and medical treatment plans; consulting with physicians and nursing personnel in administrative and treatment issues; and developing nursing standards for the Division in accordance with Standards of Developmental Disabilities Nursing Practice as outlined by the Developmental Disabilities Nurses Association.

Home Health Aides

WJCS is currently searching  for Home Health Aides,  Continuous recruitment.  10-40 hours a week in Westchester County.   Live-in positions available.  The HHA will perform routine tasks that assist patients who are essentially home-bound with normal activities of daily living in their home and provide nutritional and environmental support services, which may include housekeeping, shopping, laundry services, meal preparation, paying bills and running errands.

The HHA will provide and/or assist with personal care functions to restore and/or maintain patients' activities of daily living, i.e., bathing, grooming, feeding, self medication, skin care, equipment including walkers, crutches, canes, and oxygen. Records and/or reports care given, patient's response and any deviations from normal reactions or routines. Knowledge of specific deviation from normal physical/mental functioning. Holds a certificate from a New York State Department of Health approved training course with 60 hours of classroom and 30 hours of clinical training or an approved equivalency. Home Visitors

The WJCS Parent-Child Home Program (PCHP) is currently recruiting part-time visitors, who receive a stipend, for the new program year 2016-2017 (October – May). Staff Training will be held in September. This meaningful and exciting work requires a minimum commitment of nine hours per week. Although Home Visitors’ hours are flexible, attendance is required at weekly staff/training meetings held on Wednesday mornings at WJCS, 141 North Central Avenue, Hartsdale, NY 10530.

The Parent-Child Home Program serves low-income families in White Plains, Port Chester, New Rochelle, Mamaroneck, Greenburgh and Mt. Vernon. Visitors call on parents and children in their homes for a half hour, twice weekly, beginning when the child is about two years old. Each week, the Home Visitor brings a book or toy and in the comfort of the family’s home, parent, child and home visitor participate in fun and stimulating activities using the book or toy. The Home Visitor models verbal interaction – reading, conversation, and play – designed to develop language and literacy skills and school readiness skills.

Home Visitors need no special educational background, but must have a personality suited to home visiting and working with parents and toddler-aged children. Prior to beginning home visits, Home Visitors participate in 16 hours of paid training.

Other responsibilities include providing supervision, training, direction and evaluation of assigned clerical and support staff; ensuring proper support staffing and coverage during absences; and delegating clerical projects including photocopying, filing and computer assignments.

Registered Nurse

WJCS is searching for a  Registered Nurses. This  is full-time, 36  hours per week Under general supervision from the Director of Nursing Services, the Registered Nurse will provide nursing services for individuals residing in the homes operated by Community Programs of WJCS.

The Registered Nurse will promote optimal health and safety through assessment, planning and intervention as well as collaboration with the interdisciplinary team and other health professionals. Other responsibilities include coordinating mandated evaluations, such as physicals, dental, eye/vision and audio evaluations and provide follow up for all medical recommendations; collaborating with the IDT to promote the strengths of each individual, as well as reports on current health status at quarterly, semi annual and annual reviews; and provides training to staff on all medical protocols and ensures that all staff have a good working knowledge of the trained protocol and , ensures documentation is present for all trainings.

The ideal candidate must possess a valid license to practice as a Registered Nurse in New York State; valid driver’s license and ability to travel to various locations; have a CPR Certification; and have experience working with the ID/DD population. Residence Manager

Service Coordinator

The Service Coordinator uses a person-centered planning process to develop, implement, and maintain the Individualized Service Plan (ISP), makes referrals, conducts meetings, prepares and distributes reports as required, and ensures that Medicaid and other government benefits are maintained for assigned individuals. Social Worker

WJCS has an opening for a full-time, 35 hours per week Social Worker for our family mental health clinic in Yonkers, NY. Under general supervision of the Associate Clinic Director, the Social Worker will provide psycho-therapeutic services to children, adults and families experiencing emotional difficulties.  The Social Worker’s responsibilities consist of ongoing cases, crisis and intakes and utilizes evidence based practices. The Social Worker is responsible for all recording and statistical procedures required by the agency including case records, insurance forms, related data, monthly statistical sheets, schedules, surveys, etc; and maintaining necessary patient related contacts with other agencies and resources (courts, schools, etc.) including written material, telephone contact and personal meetings as needed.

 The ideal candidate must have thorough knowledge of modern principles and techniques of social work. Must be able to work with children, adolescents, adults or families in poorer communities, conduct interviews to obtain social and medical information necessary to perform diagnostic evaluation and be able to communicate effectively in English, both orally and in writing. Bi-lingual, English-Spanish is a plus. Knowledge of MS Office and Anazasi or other electronic medical record system is needed.

 The selected person will have a MSW from an accredited school of social work and be licensed by the New York State Education Department to practice social work. On-going clinical supervision and training is available at WJCS’ Educational Institute.

Staff Psychiatrist

WJCS, a large human service agency in Westchester County, is seeking a dynamic individual to assume the role of a psychiatrist.   The psychiatrists will privde direct care and collaboration with a multidisciplinary team working in an expanding variety of community settings.  Minimum requirement of at least two (2) years post training clinical and administrative experience, including familiarity with electronic records and Board Certification in Psychiatry.  Experience in management of child psychiatry and/or Board Certification in Child/Adolescent Psychiatry a plus, but not required.
