Care Manager, Geriatrics

WJCS is currently searching for a part-time, 14-28 hours per week Care Manager to work in our Geriatric Care Management Program based in White Plains, NY. This is a mobile position, as the Care Manager must be able to go to client’s homes. The Care Manager provides case management services for individuals participating in the program. Other responsibilities include assisting clients, both on-site and in their homes, in accessing support services, including entitlements, medical care, legal services, senior housing, educational and social programs in the community; preparing and maintaining proper records in accordance with Agency policies and procedures; and participating in community by attending meetings with other agencies serving the elderly.

 The ideal candidate must possess a Masters degree in a Human Services field or a Registered Nurse with two years experience working as a Case Manager. Must have knowledge of or proven ability to work with a geriatric population; able to establish and maintain effective working relationships and to relate well with supervisors, clients, program officials, employees and others; and computer literate. Must be able to communicate effectively in English, both orally and in writing. Candidate must have a valid driver’s license and have own vehicle.

Community Habilitation Worker

WJCS is currently searching for two part-time, up to 12 hours per week, Community Habilitation Worker, to work throughout Westchester County. Hours are flexible. Under the general supervision of the POINT Director, the Community Habilitation Worker  will be responsible to provide in home and community based habilitation services such as education, instruction, and direct services to individuals with MR/DD/LD in supported independent living settings. Other responsibilities include assisting with the development of the individuals’ Community Habilitation Plans; promoting individual’s chosen individualized outcomes that empower and enable individuals to achieve their stated goals and to live as optimally as possible; and assisting in the development of social skills by providing education, training, and role modeling.

The ideal candidate must possess a High School Diploma or equivalent with one year experience working in a related field. Experience in working with the DD population; good knowledge of nutrition and good financial practices; and able to communicate effectively in English, both orally and in writing. Candidates must maintain reliable transportation and possess a valid NYS driver’s license and a driver’s record considered acceptable to agency insurance carrier.

Direct Support Professional(s)

Community Programs of WJCS is currently searching for both full-time (40 hours/week) and part-time (12-35 hours/week) Direct Support Professionals (DSP) to work in one of the 13 group homes located in Westchester County. Under the general supervision of the Residence Manager, the DSP will support intellectually/developmentally disabled individuals in their development of basic living and social skills with the primary goal of integrating them into the community to the best of their ability. Other responsibilities include providing assistance, services and supports that promote independence and active participation in daily living within the residence and community; administering medications as assigned; and observing and reporting individuals’ illness, health care concerns, injuries, incidents and possible individual abuse.

The ideal candidate must possess a High School Diploma or equivalent. Must have knowledge of basic residential services; able to handle sensitive issues while protecting others’ welfare; and able to participate in, pass and maintain certification of WJCS required training that includes but may not be limited to Medication Administration, SCIP, CPR/First Aid, Driving the Van and Mandatory Training as identified by OPWDD. Candidates must have a valid NYS driver’s license for at least two years. A minimum 6-12 months working with people with developmental disabilities, preferred. This position requires physical activity including but limited to lifting, moving, pulling, pushing, flexing, and bending.

Home Health Aides

WJCS is currently searching  for Home Health Aides,  Continuous recruitment.  10-40 hours a week in Westchester County.   The HHA will perform routine tasks that assist patients who are essentially home-bound with normal activities of daily living in their home and provide nutritional and environmental support services, which may include housekeeping, shopping, laundry services, meal preparation, paying bills and running errands.

The HHA will provide and/or assist with personal care functions to restore and/or maintain patients' activities of daily living, i.e., bathing, grooming, feeding, self medication, skin care, equipment including walkers, crutches, canes, and oxygen. Records and/or reports care given, patient's response and any deviations from normal reactions or routines. Knowledge of specific deviation from normal physical/mental functioning. Ability to understand and carry out technical, non-technical oral and/or written directions, maturity, reliability, good physical and emotional health, communication skills. Holds a certificate from a New York State Department of Health approved training course with 60 hours of classroom and 30 hours of clinical training or an approved equivalency. Must be free from reportable disease and habituation or addiction to depressants, stimulants, narcotics, alcohol or other substances that may alter behavior. NYS Driver's License preferred.

Home Visitors

The WJCS Parent-Child Home Program (PCHP) is currently recruiting part-time visitors, who receive a stipend, for the new program year 2015-2016 (October – May).Staff Training will be held in September. This meaningful and exciting work requires a minimum commitment of nine hours per week. Although Home Visitors’ hours are flexible, attendance is required at weekly staff/training meetings held on Wednesday mornings at WJCS, 141 North Central Avenue, Hartsdale, NY 10530.

The Parent-Child Home Program serves low-income families in White Plains, Port Chester, New Rochelle, Mamaroneck, Greenburgh and Mt. Vernon. Visitors call on parents and children in their homes for a half hour, twice weekly, beginning when the child is about two years old. Each week, the Home Visitor brings a book or toy and in the comfort of the family’s home, parent, child and home visitor participate in fun and stimulating activities using the book or toy. The Home Visitor models verbal interaction – reading, conversation, and play – designed to develop language and literacy skills and school readiness skills.

Home Visitors need no special educational background, but must have a personality suited to home visiting and working with parents and toddler-aged children. Prior to beginning home visits, Home Visitors participate in 16 hours of paid training.

Personal Care Aides

WJCS is currently searching  for Personal Care Aides.  Continuous recruitment.  10-40 hours a week in White Plains, N.Y. The PCA will provide nutritional and environmental support services, which may include housekeeping, grocery shopping, laundry, meal preparation, paying bills and running errands. Provide and/or assist with personal care to maintain patients in activities of daily living (i.e., bathing, personal hygiene, toileting, self-medication, and use of durable medical equipment, ex. canes, wheelchairs, and walkers). Observe, report, and record patient affect and activities as well as care given using appropriate agency channels and records. Completion of ninth grade education or equivalent. Holds a certificate from a New York State Health Department approved training course with no less than 45 hours of classroom work or an approved equivalency. Ability to read and write English. Ability to carry out simple verbal and written directions, handle small sums of money, and ability to get along with others. Must be free from reportable disease and habituation or addiction to depressants, stimulants, narcotics, alcohol or other substances that may alter behavior.

Program Coordinator, Developmental Disabilities

WJCS  is a large, not for profit, organization with Headquarters in White Plains, NY.  We are currently searching for a full-time, 35 hours per week, Program Coordinator for the Outpatient Services for People with Developmental Disabilities Program in our Hartsdale location.  Under the supervision of the Program Director, the Program Coordinator is responsible for overseeing  various services and functions of the DD Outpatient Program as well as providing direct service to clients.  This includes coordinating intakes and case assignments, providing clinical supervision to staff and interns, preparation reports to funding sources, participation in community meetings, providing information and referral resources to families. 
 
Key elements for the candidate are: a MSW with a LCSW and 3 years of experience working with people with developmental disabilities who have co-occurring emotional problems; knowledge and experience in utilizing evidence based principals and techniques; administrative and supervisory experience.

Registered Dietitian

WJCS is currently seeking a Registered Dietitian to work on a consultant basis in our MR/DD adult residential group homes in Lower Westchester County. Under the general supervision of the Director of Clinical Services, the Nutritionist will provide nutritional services for individuals residing in the group homes operated by Community Programs of WJCS. This position will be responsible for reviewing all residents’ medical and nutritional assessments and working with an interdisciplinary team.

The ideal candidate must possess excellent clinical, communication and organizational skills and be self-directed and team-oriented. This is an excellent opportunity for a NY Registered Dietitian with 1-2 years clinical experience. Experience in working with the MR/DD population is helpful. Registered Nurse

WJCS is searching for a  Registered Nurse. The position is part-time, 30 hours per week and includes benefits.  Under general supervision from the Director of Nursing Services, the Registered Nurse will provide nursing services for individuals residing in the homes operated by Community Programs of WJCS.

The Registered Nurse will promote optimal health and safety through assessment, planning and intervention as well as collaboration with the interdisciplinary team and other health professionals. Other responsibilities include coordinating mandated evaluations, such as physicals, dental, eye/vision and audio evaluations and provide follow up for all medical recommendations; collaborating with the IDT to promote the strengths of each individual, as well as reports on current health status at quarterly, semi annual and annual reviews; and provides training to staff on all medical protocols and ensures that all staff have a good working knowledge of the trained protocol and , ensures documentation is present for all trainings.

The ideal candidate must possess a valid license to practice as a Registered Nurse in New York State; valid driver’s license and ability to travel to various locations; have a CPR Certification; and have experience working with the ID/DD population. Must possess good interpersonal and organizational skills and be able to communicate effectively in English, both orally and in writing.

Social Club Per Diem Staff

Social Worker, Bi-lingual, English/Spanish

WJCS is searching for a full-time, 35 hours per week, bi-lingual English/Spanish Social Worker to work in Peekskill. Under the general supervision of the Clinic Director or Assistant Clinic Director, the Social Worker will provide mental health services to persons in the community having psychiatric diagnosis and requiring services offered by a licensed mental health facility. Other responsibilities include providing comprehensive clinical services, including assessment, crisis intervention, and treatment to clients of all ages in a dynamic Family Mental Health clinic setting. Opportunity to work in varied treatment modalities with clients of all ages with a broad range of emotional, social and behavioral issues, On-going clinical supervision and training in the WJCS Educational Institute. LMSW required and some evenings required. Knowledge of evidence-based principles and techniques is a plus as well as comfort with computer applications and good knowledge of community resources. This position requires one evening per week.

Social Worker, Bi-lingual, English/Spanish

WJCS has an opening for a Bi-lingual, English/ Spanish, Social Worker to work in our satellite clinic in the DSS office in Yonkers, New York. This position is part time, 21 hours per week and is benefitted. Under supervision of the Program Coordinator, the social worker provides assessment, crisis intervention, referral and linkages, case coordination and treatment to clients referred by DSS. The social worker works in partnership with DSS workers and provides consultation and guidance to the DSS workers on recognition of mental health problems and related mental health issues.

The ideal candidate must have thorough knowledge of modern principles and techniques of social work: have interest and experience in working with multi-problem families; possess an excellent knowledge of community resources; and have the ability to work flexibly as part of a team, in both traditional and non-traditional settings. Bi-lingual, English/Spanish is required.

An MSW degree from an accredited school of social work and possession of or eligible for certificate to practice social work in New York State issued by the State Education Department. On-going clinical supervision and training is available in the WJCS Educational Institute.

WJCS is currently searching for a full-time or part-time, Volunteer Services Coordinator, to work in White Plains. Under the general supervision of the Director of Development, the Volunteer Services Coordinator will support the work of the Development and Public Affairs Departments. This position is responsible for recruiting, interviewing, retaining and managing volunteers. Other responsibilities include developing and executing recognition programs for volunteers and their staff supervisors; creating a community of volunteers with opportunities to meet each other and to facilitate a broader understanding of the many facets of WJCS; and supporting overall work of the Public Affairs Department by facilitating communications with government and community partners, setting up appointments with legislators, preparing informational materials and packets for meetings and presentations.

The ideal candidate must possess a Bachelors degree in Human Services or related field; experience in volunteer management; experience with MailChimp; and a high level of computer skills including social media. Previous experience in development, eTapestry and InDesign preferred. Must be able to communicate effectively in English, both orally and in writing.
