Case Manager(s)

WJCS is currently seeking several Case Managers for our Home Delivered Meals Program. This is a part-time position with flexible hours. The Case Manager will conduct various assessments of home delivered meal recipients including a financial assessment to determine eligibility for the program, services and entitlements. This position requires home visits in the Westchester County community.

The selected person must be computer literate and possess a minimum of a Bachelor's Degree in a human services field. Experience with seniors is preferred. Bi-lingual, English-Spanish speaking is a plus. The selected person needs a valid driver’s license and a vehicle as the position involves travel to recipients homes.

Community Habilitation Worker

WJCS is currently searching for two part-time, up to 12 hours per week, Community Habilitation Trainer, to work throughout Westchester County. Under the general supervision of the POINT Director, the Community Habilitation Trainer will be responsible to provide in home and community based habilitation services such as education, instruction, and direct services to individuals with MR/DD/LD in supported independent living settings. Other responsibilities include assisting with the development of the individuals’ Community Habilitation Plans; promoting individual’s chosen individualized outcomes that empower and enable individuals to achieve their stated goals and to live as optimally as possible; and assisting in the development of social skills by providing education, training, and role modeling.

The ideal candidate must possess a High School Diploma or equivalent with one year experience working in a related field. Experience in working with the DD population; go knowledge of nutrition and good financial practices; and able to communicate effectively in English, both orally and in writing. Candidates must maintain reliable transportation and possess a valid NYS driver’s license and a driver’s record considered acceptable to agency insurance carrier.

Direct Support Professional(s)

WJCS is currently searching for Direct Support Professional(s). Continuous  recruitment.   All shifts needed. Under the direction and supervision of the Residence Manager, the direct support professional will provide assistance, services and supports that promote independence and active participation in daily living within the residence and community. The residential counselor will assist the clients   to achieve their individual habitation goals and objectives. It is also necessary to provide health care and ensures the safety of the clients at all times.  

The direct support professional may administer medications as assigned. The direct support professional will serve as a role model for clients, modeling dignity and respect, drive or accompany clients to appointments, recreational, religious, day program/employment activities; and visits with others. Provide oversight and assistance to clients who are hospitalized. Complete and maintain all required documentation, records, and reports. Maintain the cleanliness and attractiveness of the residence. The selected applicant will be: a High School graduate or equivalent, have a minimum of 12 months working with people with developmental disabilities, valid driver's license, knowledge of household management, meal planning, meal preparation, shopping and knowledge of people with mental retardation, developmental disabilities, mental health challenges, and health care conditions. The ability to communicate effectively in English, both orally and in writing, capable  to physically assist consumers, flexible schedule and ability to work in a variety of locations and on different shifts/time periods. It is preferred that this staff person has the ability to pass and maintain certification of WJCS required training that includes, but may not be limited to Medication Administration, SCIP, CPR/First Aid, vehicle certification and Mandatory Training identified by OPWDD.

Home Health Aides

WJCS is currently searching  for Home Health Aides,  Continuous recruitment.  10-40 hours a week in Westchester County.   The HHA will perform routine tasks that assist patients who are essentially home-bound with normal activities of daily living in their home and provide nutritional and environmental support services, which may include housekeeping, shopping, laundry services, meal preparation, paying bills and running errands.

The HHA will provide and/or assist with personal care functions to restore and/or maintain patients' activities of daily living, i.e., bathing, grooming, feeding, self medication, skin care, equipment including walkers, crutches, canes, and oxygen. Records and/or reports care given, patient's response and any deviations from normal reactions or routines. Knowledge of specific deviation from normal physical/mental functioning. Ability to understand and carry out technical, non-technical oral and/or written directions, maturity, reliability, good physical and emotional health, communication skills. Holds a certificate from a New York State Department of Health approved training course with 60 hours of classroom and 30 hours of clinical training or an approved equivalency. Must be free from reportable disease and habituation or addiction to depressants, stimulants, narcotics, alcohol or other substances that may alter behavior. NYS Driver's License preferred.

Home Visitors

The WJCS Parent-Child Home Program (PCHP) is currently recruiting part-time visitors, who receive a stipend, for the new program year 2015-2016 (October – May).Staff Training will be held in September. This meaningful and exciting work requires a minimum commitment of nine hours per week. Although Home Visitors’ hours are flexible, attendance is required at weekly staff/training meetings held on Wednesday mornings at WJCS, 141 North Central Avenue, Hartsdale, NY 10530.

The Parent-Child Home Program serves low-income families in White Plains, Port Chester, New Rochelle, Mamaroneck, Greenburgh and Mt. Vernon. Visitors call on parents and children in their homes for a half hour, twice weekly, beginning when the child is about two years old. Each week, the Home Visitor brings a book or toy and in the comfort of the family’s home, parent, child and home visitor participate in fun and stimulating activities using the book or toy. The Home Visitor models verbal interaction – reading, conversation, and play – designed to develop language and literacy skills and school readiness skills.

Home Visitors need no special educational background, but must have a personality suited to home visiting and working with parents and toddler-aged children. Prior to beginning home visits, Home Visitors participate in 16 hours of paid training.

Learning Specialist

WJCS is currently searching for an energetic, Learning Specialists  to perform part-time tutorial work with children who are experiencing difficulties with high school level math.  This position can be successfully filled by an individual who has attained or is in the process of working toward completion of a Bachelor's degree or higher in education, special education, math, reading, psychology or other liberal arts. Hourly salary is commensurate with education and experience. The applicant should have excellent communication skills in order to provide an exceptional learning experience for the student.  Tutors must be available to work after school hours and on Saturdays.   Bi-lingual, English-Spanish, is a plus but ability to work with students on Algebra, Geometry, Trigonometry and Precalculus skills is mandatory.

Personal Care Aides

WJCS is currently searching  for Personal Care Aides.  Continuous recruitment.  10-40 hours a week in White Plains, N.Y. The PCA will provide nutritional and environmental support services, which may include housekeeping, grocery shopping, laundry, meal preparation, paying bills and running errands. Provide and/or assist with personal care to maintain patients in activities of daily living (i.e., bathing, personal hygiene, toileting, self-medication, and use of durable medical equipment, ex. canes, wheelchairs, and walkers). Observe, report, and record patient affect and activities as well as care given using appropriate agency channels and records. Completion of ninth grade education or equivalent. Holds a certificate from a New York State Health Department approved training course with no less than 45 hours of classroom work or an approved equivalency. Ability to read and write English. Ability to carry out simple verbal and written directions, handle small sums of money, and ability to get along with others. Must be free from reportable disease and habituation or addiction to depressants, stimulants, narcotics, alcohol or other substances that may alter behavior.

Program Coordinator

WJCS  is a large, not for profit, organization with Headquarters in White Plains, NY.  We are currently searching for a full-time, 35 hours per week, Program Coordinator for the Outpatient Services for People with Developmental Disabilities Program in our Hartsdale location.  Under the supervision of the Program Director, the Program Coordinator is responsible for overseeing  various services and functions of the DD Outpatient Program as well as providing direct service to clients.  This includes coordinating intakes and case assignments, providing clinical supervision to staff and interns, preparation reports to funding sources, participation in community meetings, providing information and referral resources to families. 
 
Key elements for the candidate are: a MSW with a LCSW and 3 years of experience working with people with developmental disabilities who have co-occurring emotional problems; knowledge and experience in utilizing evidence based principals and techniques; administrative and supervisory experience.

Registered Nurses

WJCS is searching for two (2) Registered Nurses. The position is part-time, 30 hours per week and includes benefits.  Under general supervision from the Director of Nursing Services, the Registered Nurse will provide nursing services for individuals residing in the homes operated by Community Programs of WJCS.

The Registered Nurse will promote optimal health and safety through assessment, planning and intervention as well as collaboration with the interdisciplinary team and other health professionals. Other responsibilities include coordinating mandated evaluations, such as physicals, dental, eye/vision and audio evaluations and provide follow up for all medical recommendations; collaborating with the IDT to promote the strengths of each individual, as well as reports on current health status at quarterly, semi annual and annual reviews; and provides training to staff on all medical protocols and ensures that all staff have a good working knowledge of the trained protocol and , ensures documentation is present for all trainings.

The ideal candidate must possess a valid license to practice as a Registered Nurse in New York State; valid driver’s license and ability to travel to various locations; have a CPR Certification; and have experience working with the ID/DD population. Must possess good interpersonal and organizational skills and be able to communicate effectively in English, both orally and in writing.

Social Worker

WJCS is currently searching for a part-time, 35 hours per week Social Worker to work in our Mount Vernon, NY location.  Under general supervision of the Clinic Director, the Social Worker will provide psycho-therapeutic services to a diverse group of individuals experiencing emotional difficulties in the community.  Other responsibilities include, providing clinical services to children, adults, and families utilizing various therapeutic modalities, including ongoing cases, crisis and intakes, using evidence based practices; responsible for all recording and statistical procedures required by the agency including case records, insurance forms and related data, monthly statistical sheets, schedules, surveys, etc; and maintaining necessary patient related contacts with other agencies and resources (courts, schools, etc.) including written material, telephone contact and personal visits as necessary.

 

The ideal candidate must have thorough knowledge of modern principles and techniques of social work.  Must be able to work with children, adolescents, adults or families.  Must be able to conduct interviews to obtain social and medical information necessary to perform diagnostic evaluation.  Able to communicate effectively in English, both orally and in writing.  Knowledge of Anazasi or other Electronic Medial Record System.  Must have a Masters Degree in Social Work from an accredited school of social work and licensed by the New York State Department of Education.

Social Worker, Bi-lingual, English/Spanish

WJCS has an opening for a Bi-lingual, English/ Spanish, Social Worker to work in our satellite clinic in the DSS office in Yonkers, New York. This position is part time, 21 hours per week and is benefitted. Under supervision of the Program Coordinator, the social worker provides assessment, crisis intervention, referral and linkages, case coordination and treatment to clients referred by DSS. The social worker works in partnership with DSS workers and provides consultation and guidance to the DSS workers on recognition of mental health problems and related mental health issues.

The ideal candidate must have thorough knowledge of modern principles and techniques of social work: have interest and experience in working with multi-problem families; possess an excellent knowledge of community resources; and have the ability to work flexibly as part of a team, in both traditional and non-traditional settings. Bi-lingual, English/Spanish is required.

An MSW degree from an accredited school of social work and possession of or eligible for certificate to practice social work in New York State issued by the State Education Department. On-going clinical supervision and training is available in the WJCS Educational Institute.
