Geriatric Outreach Services

Through this program, older individuals (minimum age 55) and their families receive a wide array of mental health services, comprised of individual, marital, family and group psychotherapy, psychiatric consultation, medication monitoring and case management.   Experienced social workers help clients cope with issues related to growing older in our society, including maintaining independence in the face of loss and changing family structure, retirement, physical illness or disability, isolation from family and friends, and continuing responsibility for dependent adult children, grandchildren, ill spouses or elderly parents. The outreach component of our program enables our staff to reach out to those individuals who are homebound by providing treatment and case management in the home. 

A constantly updated knowledge base of community resources also enables the case manager to assist individuals in obtaining entitlements, assistance and services, as appropriate, to increase the viability of our clients in the community.
