WJCS is currently searching for a full time, 35 hours/week, Care Manger to work as a part of an innovative, exciting project supported by NYS Office of Mental Health. The NYS System of Care High Fidelity Wraparound Project (HFW) is a project grounded in principles, a framework and core values within a System of Care. HFW is an evidence-based process that support youth with severe emotional and behavioral needs. The project provides team-based planning, that is comprehensive, well-coordinated, holistic and family/youth-driven, with the intent to meet the complex needs of youth with serious mental health needs. The goal is to bring all parties to the planning table, to support a plan that is responsive to the family’s identified needs. Training is extensive and designed to lead the Care Manage to a NYS Certification in High Fidelity Wraparound.
The ideal candidate must possess a Bachelors Degree with two years experience working with children/adolescents or a Master’s Degree with one-year experience; ability to work with youth/adolescents/young adults from 5 to 21 years old; meeting facilitation skills, strong collaboration skills; ability to engage and work with parents/caregivers. A driver’s license and vehicle are required. Bi-lingual, English-Spanish is a plus.
